Help:Unresponsive Profile Managers

I have not been able to find the Unresponsive Profile Manager Request Form.  Having read the "Help: Unresponsive Profile Manager" page and followed the directions as best i can and made several attempts to locate this form with no success, I would like some future explanation as to where it is or if it has some other title.  

Is there a simple route to this form?  Again I have followed the directions with no positive result.

Click the Unresponsive Profile Manager Request Form link in the Contact section of the Tree & Tools page of the profile.

As this profile is privacy locked there is no tree and tool page available.  Is there a alternate method  to get there?

Email  and explain what steps you have  taken.

You can find the UPM request form link at the bottom right corner of the orange box that says "Only the Trusted List can access ... "

Hi James,

If you are trying to gain access to or have questions about a specific profile for which the profile manager is not responding, select the "Family Tree and Tools" tab on that profile and scroll to the bottom of the page. There you should find the link for the "Unresponsive Profile Manager Request Form".

Make sure that you are on the profile in question (not the profile manager's own profile).
