Need help with a German/Polish Jewish birth record from Tarnopil, Galicia 1884

Hi, I was wondering if someone could help me extract some more information from what I think looks like the birth record for my great-grandmother, Yetta Weiss [Weiss-3223].  On her marriage record (https://a860-historicalvitalrecords.nyc.gov/view/8238622) her name was listed "Mary Zipper", and her parents as "David Zipper" and "Nessie Weisshaut".  In other records went by Yetta or Etty, and mostly with the surname Weiss or her married name: Fischman/Fishman.

She was from Tarnopil, Galicia (Austria/Poland/Russia/Ukraine) born in 1884.  According to the jr-poland.org website, they have indexed the birth record as:

Can someone figure what the rest of the info and names are in that record?  Thanks.

You have two generations of non-civil marriages. It appears that both couples were married religiously, however they did not apply/purchase the Austrian civil marriage license, thus the "out of wedlock" status for their children.

Thank you so much!  So might that be why Marjim Ettel switches around between Weiss and Zipper?  When you say two generations, are you thinking that (?) Wolf and Chaje Weisshaut were also did not have a civil marriage?
