The affidavit is a request for a Delayed certificate of birth due to lost original certificate of birth. How it became joint aff, of the Parents I do not know, joint was typed in by the employees in the office. All Information written in is my dads hand writing. Sadie Martin Roland my grand mom did die in 1918. I am 76 and looking for her Burial site if you know email me [email address removed] ( today is 11-23-15)
