Frederick Arthur Slade was born in Salt Lake City, Utah Territory on December 11, 1864. He was the son of Henry Slade and Emily Clifton .
Frederick died in Dallas, Texas on December 23, 1946, and was buried in Forest Lawn Memorial Park, Long Beach, California[1] .
