Whilst the Regiment was making a charge on the aforesaid Fort Wagner, the said Andrew Clark was shot through the thigh with a mini-ball and immediately died on the 18th day of July 1863 as aforesaid. That she was married to the said Andrew Clark on the 15th

On this 26th day of September, 1865, before me Prothonotary of the Court of Common Pleas in and for the county and State above named personally appeared Rebecca Clark, a resident of the township of West Whiteland in the State of Pennsylvania aged 45 years who being duly affirmed according to law declares that she is the widow of Andrew Clark who was a private in company "B" [research note: he served in Co. D] commanded by Captain Newell of the 54th Regiment of Massachusetts volunteers commanded by Colonel Shaw and who was killed on the 18th day of July 1863 at Fort Wagner S[outh] C[arolina] while in the service of the United States.
