Might be Waters of the Pa? Although it doesn't look like a p, the first letter looks similar to the p in petitioner, particularly the curl at the top left that many other letters don't seem to have. Might be worth looking at some old maps of a county/state to see if theres anything that looks right.
For the last row of blanks, my best guess is "ad m duly bond (something still indecipherable)" perhaps duly bound? That sound legal-ish.
