Birth
Hendrika Adolphina was born on the 9th of June, 1906, in Utrecht to Antonie Cornelis de Nijs and Odilia Fritz[1].
The translation of her birth certificate reads as follows:
On this day the eleventh of June, nineteen hundred and six, appeared in the House of the Municipality in the presence of the undersigned, Registrar of Births, Deaths and Marriages of the city of Utrecht:
Antonie Cornelis de Nijs, railroad official, thirty-four years of age, residing in the Weteringstraat, who declared to us, that on the evening of ten o'clock on the ninth of this month his lawful wife, Odilia Fritz, without profession gave birth at his residence to a child of the female gender, who shall be given the first name Hendrika Adolphina.
This declaration was made in the presence of Carl Furchtegott Bles, aged thirty-five years, and Johannes van Engelenburg, aged fifty-eight years, messengers, both living here, of which we have taken up this deed and read it to the declarant and the witnesses, who have signed with us.

Death
Hendrika passed away on the 23rd of July, 1906, in Utrecht at the age of about a month and a half[3].
The translation of her death certificate reads as follows:
On this day the twenty-fourth of July, nineteen hundred and six, we, the undersigned Registrar of Births, Deaths and Marriages of the city of Utrecht, appeared in the house of the municipality:
Gerardus van der Veer, undertaker's assistant, aged forty-seven years, and Johannes Engelenburg, messenger, age fifty-seven years, both residing here, who declared to us that on the twenty-third of this month, at nine o'clock in the morning, in the age of one month, in this city, died:
Hendrika Adolphina de Nijs, born in Utrecht, daughter of Antonie Cornelis de Nijs, worker, and Odilia Fritz, without profession, both living in Utrecht at the Weteringstraat.
